In certain clinical situations, periradicular lesions of endodontic origin can be difficult to distinguish from lesions caused by periodontal disease. When a sinus tract originating from an endodontic lesion drains along the periodontal ligament, the radiographic and clinical presentation may closely resemble a periodontal pocket.
Cone beam computed tomography (CBCT) may reveal extensive periradicular bone destruction; however, it does not provide definitive information regarding remaining soft tissue attachment. Therefore, accurate diagnosis requires integration of clinical findings, radiographic interpretation, pulp testing, periodontal evaluation, and adjunctive imaging.
This lecture will review the biological mechanisms of bone lesion formation secondary to pulpal disease, including pathways of extension into the oral cavity. Emphasis will be placed on diagnostic decision-making and clinical management of endodontic–periodontal and large periradicular lesions.
Learning Objectives:
- Understand the diagnosis, treatment options, and prognosis of apical periodontitis when it mimics periodontal disease.
- Recognize the causes of non-healing endodontic lesions that present with periodontal characteristics.
- Differentiate between endodontic and periodontal lesions, including lesions associated with cracked and fractured teeth.
- Formulate evidence-based treatment strategies for “J-shaped” and large radiolucencies.
- Demonstrate the prognosis of treating these types of periradicular lesions.
Jorge Vera, DDS, MS, is a Clinical Assistant Professor and Director of the Advanced Education Program in Endodontics in the Department of Periodontics and Endodontics at the University at Buffalo School of Dental Medicine. Dr. Vera is also a Diplomate of the American Board of Endodontics. Dr. Vera received his Postgraduate Certificate in Endodontics from Tufts University School of Dental Medicine in 1993. In 2005, he was the recipient of the Samuel Seltzer Award from Oral Surgery, Oral Medicine, Oral Pathology, Oral Radiology, and Endodontology.
He serves on the Scientific Advisory Boards of the Journal of Endodontics and the International Endodontic Journal. Dr. Vera has lectured in more than 40 countries, including presentations at meetings of the European Society of Endodontology and the American Association of Endodontists. He has authored more than 80 endodontic publications and contributed to three book chapters. Dr. Vera maintained a private practice limited to Endodontics from 1993 to 2020.
